Leaving Peyronie’s disease untreated does not affect every man in the same way, but for some, symptoms can progress beyond curvature alone and begin affecting erections, intimacy and emotional wellbeing.

One of the biggest misconceptions about Peyronie’s disease is that it is only a cosmetic issue involving a bent erection.

In reality, the condition can affect much more than curvature alone. For some men, symptoms remain mild or stabilise. For others, changes in shape, erections, intimacy and confidence may become more noticeable over time.

This does not mean every untreated case will worsen. Progression varies significantly between individuals. However, understanding what can happen over time may help you decide whether monitoring or specialist assessment would be useful.

Penile Curvature May Become More Noticeable

For some men, curvature may become more pronounced as Peyronie’s disease develops.

A mild bend that initially seems manageable can sometimes become more noticeable, particularly if plaque formation changes how the penile tissue stretches during erection.

Why Curvature Can Change

Peyronie’s disease involves scar tissue, often called fibrous plaque, forming within penile tissue.

This plaque can reduce elasticity and restrict blood flow in one area. During erection, the less flexible section may not expand normally, which can cause bending or pulling toward the affected side.

As tissue changes, curvature may also change.

Changes in Severity Over Time

Progression is not always dramatic.

Some men notice only small structural changes. Others find that curvature gradually begins affecting sexual comfort or penetration.

For example, a man may initially notice a slight upward bend that causes little concern. Months later, the curve may become more pronounced and start interfering with intercourse.

In some men, changes can also feel sudden, with a noticeable increase in curvature or a new deformity appearing over a short period, sometimes within days rather than months.

Curvature Is Not the Only Structural Change

Curvature is only one part of the picture.

Peyronie’s disease may also involve:

  • penile shortening
  • narrowing
  • indentation
  • hourglass deformity
  • reduced rigidity in part of the shaft

These structural changes are often overlooked, yet they can affect function as much as curvature itself.

If you want a broader overview of the condition, our guide on what Peyronie’s disease is explains the underlying process in more detail.

Pain Can Continue or Change Over Time

Pain may improve for some men, while others continue to experience discomfort or notice changes in symptoms.

Pain is often more common during the active phase of Peyronie’s disease, when tissue changes are still occurring.

Some men experience:

  • pain during erection
  • tenderness when touching plaque
  • discomfort during sex

For some, pain settles as the condition becomes more stable. For others, discomfort remains part of the condition or changes in character over time.

Pain alone does not reliably indicate severity. This is one reason symptoms are best understood in the overall context rather than in isolation.

Erections May Become More Difficult

Some men experience increasing difficulty achieving or maintaining erections as the condition develops. In some cases, fibrous plaque may alter the normal expansion of penile tissue and affect blood trapping during erection, which can contribute to reduced rigidity.

Erectile difficulties can arise from both physical and psychological factors.

Physical Factors

Structural changes may affect rigidity or make erections mechanically less stable.

Plaque-related changes can also contribute to reduced erection quality in some cases.

Psychological Factors

Anxiety can also play a major role.

Men who worry about pain, shape changes or sexual performance may become increasingly self-conscious during intimacy. That stress can itself contribute to erection difficulties.

How Erectile Difficulties Can Affect Confidence

When erection changes begin alongside curvature, emotional impact often increases.

Men may begin questioning:

  • whether intercourse will be possible
  • whether symptoms are worsening
  • whether treatment is needed

Understanding available Peyronie’s disease treatment options can help provide context when symptoms begin affecting function.

Sexual Confidence Can Be Affected

The emotional effects of Peyronie’s disease can sometimes become as significant as the physical symptoms and are often underestimated.

Even when symptoms remain physically manageable, many men report growing anxiety around intimacy.

Common emotional effects include:

  • self-consciousness
  • embarrassment
  • fear of partner reaction
  • avoidance of sexual situations
  • reduced confidence

A man may still be physically capable of intercourse but avoid initiating intimacy because he feels ashamed or anxious about how his penis looks or functions.

This emotional burden can gradually become one of the most difficult parts of living with the condition.

Intimacy and Relationships May Become Strained

Changes in sexual confidence and function can affect communication and intimacy within relationships.

Peyronie’s disease rarely affects only the individual.

Avoidance Behaviours

Some men withdraw physically or emotionally to avoid difficult conversations.

This may look like:

  • avoiding sex
  • reduced affection
  • making excuses to avoid intimacy

Partners may misinterpret this as rejection or loss of interest.

Communication Challenges

Many men struggle to explain what they are experiencing.

Embarrassment often leads to silence, even in long-term relationships, and without communication, misunderstanding can grow and become a catalyst for knock-on effects.

Impact on Partners

Partners may also feel confused, worried or unsure how to help.

Supportive communication can make a meaningful difference, especially when both partners understand that the condition involves more than visible curvature.

Treatment Options May Become More Limited

As symptoms change, some treatment pathways may become more appropriate than others.

This does not mean treatment becomes impossible if you wait.

It does mean the most appropriate treatment approach may differ depending on disease stage.

Why Timing Can Matter

Treatment decisions are influenced by:

  • symptom progression
  • disease stability
  • severity of curvature
  • erectile function
  • patient goals

Timing can affect which discussions are most relevant.

Active Versus Stable Disease

Peyronie’s disease is often considered in two broad phases.

Active phase – Symptoms may still be changing. Curvature, pain or plaque may evolve.

Stable phase – Symptoms have become more consistent, with little recent change.

Understanding which phase you may be in can help guide treatment discussions.

Importance of Assessment

Assessment does not commit you to treatment. Often, it simply provides clarity.

If you are unsure whether symptoms are changing or want to better understand your options, you can contact us for further guidance.

Peyronie’s disease, left untreated, does not follow the same path for every man.

For some, symptoms remain relatively stable. For others, changes may extend beyond curvature and affect penile shape, erections, sexual confidence and relationships.

This is why Peyronie’s disease is best understood as more than a structural bend alone.

If you are unsure whether your symptoms are changing, early assessment can help clarify whether you are in the active or stable phase and which treatment discussions may be relevant.
